We assigned patients and their families to skilled case managers to help them through hospice care coordination. We work closely with the patients and develop a specialized care plan aimed at improving their quality of life and enabling them to freely live their final days comfortably.
Patients who suffer from severe illness and disease often suffer the pain of their symptoms and are unable to live their life comfortably. For patients who wish to end medical care and transition to end-of-life care, we help them manage their symptoms so that as much as possible we can ensure their final days are spent comfortably.
Our health professionals work closely to evaluate the patient’s condition, monitor symptoms and from there develop a specialized care plan geared towards managing their pain and improving their quality of life.
